Before a visit, a carer has minutes, not hours, to understand a citizen. We reorganised the overview so alerts such as allergies and fall risk sit right next to the name, the conditions with the highest severity come first, and today’s visits and services are always in view.

The tabs for conditions, services, notes and measurements follow the same structure, so what you learn in one tab applies in the next. Measurements outside the normal range are marked with both colour and text, and tell the carer what to do next.

Home care planning changes all day: sick days, new citizens and visits that take longer than planned. When someone calls in sick at 06.45, the planner has to see the consequence at once.

We designed a planning view where visits without a carer move to their own lane, overlaps are marked instead of hidden, and a suggested plan hands the visits to colleagues with room. The planner stays in control and approves or adjusts the suggestion before it goes out.

Out on the route, the phone has to work with one hand, in poor light and in the few minutes between two visits. So the day is shown as a route, with the next visit at the top.

One tap opens the visit with the alerts first, the tasks as a checklist and a note that goes straight into the record. When the visit is finished, the record is updated for the next colleague, without anyone having to type it in again at the office.
